Rate each item from 0–3 and write your score on the line beside it.

0 = Never/Rarely |

1 = Occasionally/Slightly |

2 = Frequently/Moderately |

3 = Constantly/Severely

  • I experience low mood, irritability, or hopelessness. _______
  • I feel chronically fatigued and rest doesn’t help. _______
  • I eat meals at irregular times. _______

🧠 Interpreting Your Results

This self-assessment isn’t a medical diagnosis, but it offers powerful insight into the state of your nervous system, your adrenal function, and your emotional and physical resilience.

If you score:

  • 0–10: You may be showing early signs of stress overload. Now is a great time to take preventative steps.
  • 11–and above: Any result above 11 warrants some concern, and indicates your autonomic nervous system needs help! Please make an appointment to see your local doctor!
